All forms are available in .pdf format. If you have trouble printing them, you can obtain the forms in person by visiting the Intramural Office, 2nd floor room 208, Varsity Pavilion.

Team Entry forms are required by the Intramural Office for every team. Please hand in the form to the Main Office or Intramural Office.

Every team is required to have their Player Registration form handed in prior to their second league game. Failure to do so could lead to a forfeited game. All participants must sign the player registration list PRIOR to him/her playing their first game.

|
Players' Pool form is for those who wish to sign up as an individual. When completed, please drop your form off at the Intramural Office, where it will be placed in a binder that is available to every captain. Please note that the Intramural Office will not place an individual on a team.

Playoff Eligibility forms are due before your first playoff game. Please refer to the Intramural Office for specific playoff eligibility requirements for your sport.

|
The Intramural T is presented to a student in his/her graduating year. The "T" shall be awarded based on an individual's contributions to the Intramural Program, as a player, coach/manager, Official, Administrator, or Committee/Council member. Nomination Forms are due to the Intramural Office by mid February.
